New Clients: Important information regarding your first appointment
On this page I've included all the information you need to know about your first appointment, including links to paperwork that needs to be completed before and brought to your first appointment. All the necessary forms are linked off this page and will open when you click on them.

office location & parking
My warm and inviting Cochrane office is located in the scenic town of Cochrane, Alberta. Give yourself some extra time to get to your first appointment so you don't arrive rushed. Click here for a Google map.
The office is located in Cochrane at 70 Railway Street East, Unit #4. Cochrane can be reached from Calgary via either Highway 22 or Highway 1A.
There are two free parking spots in front of the building door and plenty of free parking on and across the street. The office is on the main floor.
When you arrive please have a seat in the waiting room located behind reception, relax, and help yourself to any refreshments or snacks and I will come out and greet you when it is time for your session to begin.
NW Calgary Office
The NW Calgary office is located on Varsity Estates Drive just in front of the Silver Springs Golf Course. When driving look for the Varsity Executive Business Centre. Give yourself some extra time to get to your first appointment so you don't arrive rushed. Click here for a Google map.
There is lots of free parking at the back of the building or at the golf course. It's best to drive to the back of the building or - if you park at the front of the building - to still walk all the way around to the back of the building for access. *Note: For easier and more direct access to the office, it's best to avoid parking at the front of the building facing Varsity Estates Drive as this is a separate entrance with less direct access to the office. Please do not park in the angled visitor parking directly on the side of the building right outside the unit doors as this is private parking.*
If your appointment is scheduled for before 5pm on a weekday please walk around to the back of the building where the Varsity Executive Business Centre is located and go through their main entrance and let the receptionist know you're here to see Dr. Easter Yassa and what time your appointment is scheduled. The receptionist will then call me to let me know you're here and will either walk you back to our offices or I will come to the waiting room at our scheduled appointment time to meet you and we'll head on back to my office.
If your appointment is scheduled for 5pm or later or on a weekend then you'll come to the side of the building at the glass doors and buzz #101. This will ring inside letting me know you're here. If you're here prior to your appointment time please return to your car and relax until your appointment time and at your scheduled appointment time I will come to the glass doors to let you in and we'll head on back to my office.
Once we arrive in my office, the first thing we will take care of is payment. Payment information can be found here. Unless otherwise discussed the fee is $200 per hour.
Payment forms are cash, cheque (made out to Dr. Easter Yassa), online Interac eTransfer* via your bank, VISA, AMEX or Mastercard.
(*If choosing eTransfer please make the answer to the security question IMATTER.)
Sessions that extend past the allotted time will be charged in 15 minute increments and payment for the additional time will be taken care of at the end of the session.
You will need to complete, sign and bring with you to the first session this Standing Credit Card Authorization Form, which will be kept on file. If you have questions or concerns about this please ask during the first session.
After we have taken care of payment, we will then turn our attention to necessary forms.
When we meet I will need to go over the Consent Form with you and both of us will sign it.
The more familiar you are with both the above forms the more quickly we can move through their content and get to talking about the things that matter to you.
You will also need to print off and fill out the Client Information Sheet and bring it completed to the first appointment. After the above forms have been completed I will ask you to give me your completed Client Information Sheet. This handout involves information that is needed for your file as well as helping you mentally prepare for the session. This form takes about 10-15 minutes to complete. I'd much rather spend the time we have talking to you than filling a form so please fill it out ahead of time.
Online counselling by video conference
Please note that all new clients must attend an in-person session first before booking an online video conference session. It's important to know that online video conference sessions are not a fit for every issue, every therapy approach, or every person so it's important to discuss this with me before choosing this option so we can make sure it is a fit for you and your therapy needs.
If you are an existing client and have signed up for an online video conference session please make sure to complete and submit the Consent Form for Communication or Session by Online Video Conference after having reviewed any questions/concerns you may have about it with me and prior to your scheduled appointment.
Payment for online sessions using video conference is due at the beginning of the session and receipts can be provided by email or fax depending on your preference. Payment can be made by eTransfer or credit card for online sessions.You will also need to complete this Standing Credit Card Authorization Form, which will be kept on file.
